I found only one error in displ_survey.php Change survey insted servery in line <center><iframe src="servey/poll-multi-choice/survey.php" width="750" height="425" scrolling="yes" frameborder="0"></iframe>
Just some feedback on your installation code. There are many places where the word "servey" should be changed to "survey. This causes the displays to be wrong and then also from the website link in the footer it takes the wrong path as the path includes the name "servey" and the path should include "survay". Should these be fixed the installation should be flawless.
Some more suggestions. It would nice if you can include the changes that will have to be done, if the adminModern theme is used in admin.
